Required 11th grade Physicals
The Pennsylvania School Health Code requires all students entering the 11th grade to have a physical examination report on file in each student's school health record.
Acceptable Physicals:
- 10th grade physical
- 11th grade physical
- Sports physical (10th or 11th grade)
- Proof of driver's permit or license
- Physical Exam Form
